Child Sleep Problems / Night Seizures

Night terrors, sleep walking, repetitive stereotyped movements during sleep, or morning fatigue with memory gaps may indicate nocturnal seizures requiring EEG evaluation.

Some epileptic seizures occur exclusively or predominantly during sleep, making them invisible to parents. Nocturnal frontal lobe epilepsy causes stereotyped movements, cycling of legs, or vocalizations during sleep. Children may wake with unexplained wetness (urinary incontinence during a seizure), extreme fatigue, or no memory of the night. An overnight or sleep-deprived Video EEG is the key diagnostic tool.

Warning Signs to Watch

  • Stereotyped, repetitive movements during sleep (cycling legs, rocking, vocalizing)
  • Wakes screaming and confused, then returns to sleep with no memory
  • Morning extreme fatigue, headache, or muscle soreness after apparent normal sleep
  • New onset bedwetting in a previously toilet-trained child

  • Parental video recording of episodes during sleep

Can seizures happen only during sleep?

Yes. Some epilepsy syndromes — particularly nocturnal frontal lobe epilepsy — cause seizures almost exclusively during sleep. The child may have no daytime episodes at all. A parent may notice only that the child is excessively tired in the morning or seems to have had a rough night. An overnight EEG is the only way to diagnose this.
